Lauri Markkanen scored a career-high 31 points and hit a three-pointer to give the Cavs a 117-114 lead with 28.3 seconds left in overtime.
Nuggets guard Bones Hyland missed a would-be, game-tying three-ball at the buzzer.
The Nuggets had a handle on the game in the fourth quarter. They led 92-78 after DeMarcus Cousins drove to the basket and scored with 10:29 to go.
But the Cavs battled back thanks to a 24-10 run and led 102-100 with 4:20 to go.
Nuggets center Nikola Jokic hit a short jumper with 43.2 second left in the regulation to tie the game at 107 before the teams went to overtime.
Jokic finished with 32 points, 10 rebounds and eight assists. Aaron Gordon had 20 points, eight rebounds and five assists.
The Nuggets team-record seven game road winning streak came to an end.
